Community-Based Integrated Care Fellowship Program Curriculum

As part of the community-based integrated care fellowship, each participant will participate in the following areas:
 
Online Didactics
 
Systematic didactic instruction will be provided online as one component of the fellowship. This example didactics schedule indicates curriculum topics for the community-based program.
 
In-person Didactics
 
In-person didactics are used for learning that requires more synchronous activity, including group exercises, skills practice, and building a community of learners.
 
Dates for the current cohort are the following Mondays and Tuesdays:
 
March 2-3, 2020
June 1-2, 2020
September 28-29, 2020
February 22-23, 2021
 
Mentorship
 
Participants will be matched with a UW Integrated Care Training Program faculty member for monthly mentorship sessions.
 
Quality Improvement (QI)
 
Each participant will plan a quality improvement project to support the delivery of collaborative care principles in his or her current practice setting. Program support for this project will be provided in monthly QI calls as a cohort, QI core curriculum, monthly online assignments related to the QI project, and four in-person QI sessions.
