As part of the community-based integrated care fellowship, each participant will participate in the following areas:
Systematic instruction will be provided as a combination of self-paced distance learning and quarterly 2-day in-person sessions. This example didactics schedule includes likely curriculum topics for the community-based program. A more complete schedule, including dates and faculty instructors, is being finalized.
The self-paced portion requires a weekly commitment of approximately four hours.
We anticipate the in-person sessions will be held in Seattle. Dates for these sessions will be confirmed and available soon.
Participants will be matched with a UW Integrated Care Training Program faculty member for monthly mentorship sessions.
Quality Improvement (QI)
Each participant will plan a quality improvement project to support the delivery of collaborative care principles in his or her current practice setting. Program support for this project will be provided in monthly QI calls as a cohort, QI core curriculum, monthly online assignments related to the QI project, and four in-person QI sessions.
