Community-Based Integrated Care Fellowship Program Curriculum

As part of the community-based integrated care fellowship, each participant will participate in the following areas:
 
Didactics
 
Systematic instruction will be provided as a combination of online distance learning and quarterly 2-day in-person sessions, to be held in Seattle. This example didactics schedule indicates curriculum topics for the community-based program.
 
The online portion requires a weekly commitment of approximately four hours.
 
Anticipated dates for the 4 in-person sessions for the March 2019 cohort are the following Mondays and Tuesdays:
 
March 4-5, 2019
June 3-4, 2019
September 23-24, 2019
February 24-25, 2020
 
Mentorship
 
Participants will be matched with a UW Integrated Care Training Program faculty member for monthly mentorship sessions.
 
Quality Improvement (QI)
 
Each participant will plan a quality improvement project to support the delivery of collaborative care principles in his or her current practice setting. Program support for this project will be provided in monthly QI calls as a cohort, QI core curriculum, monthly online assignments related to the QI project, and four in-person QI sessions.
