Community-Based Integrated Care Fellowship Program Goals

By the end of this program, the psychiatric consultant will be able to:
 
  1. Demonstrate the core skills of a psychiatric consultant as part of a Collaborative Care team
  2. Plan and execute a quality improvement project to apply Collaborative Care principles in current practice setting
  3. Describe the core features of implementation of an integrated care program to deliver mental health services in a primary care setting, including developing new workflows
  4. Articulate the rationale (including citing evidence from the literature) and business case for developing integrated care practices to leverage scarce psychiatric services
  5. Advocate for effective integrated care practices and educate interdisciplinary team members
  6. Promote personal and organizational well-being and resilience through a commitment to continuing education, participation in professional networks, and practice improvement
