Community-Based Integrated Care Fellowship Program Curriculum

As part of the community-based integrated care fellowship, each participant will participate in the following areas:

Online Didactics

Systematic didactic instruction will be provided online as one component of the fellowship. This example didactics schedule indicates approximate curriculum topics for the community-based program. 

Live Skills-Based Training

In-person didactics are used for learning that requires more synchronous activity, including group exercises, skills practice, and building a community of learners.

Dates for the 2024-2025 cohort are the following Mondays and Tuesdays:

September 23-24, 2024

January 13-14 2025

April 21-22, 2025

June 23-24, 2025

Dates for the 2025-2026 cohort will be posted once they are available. 

These sessions are typically held in Seattle, but were moved to Zoom due to COVID restrictions last year as needed.

Mentorship

Participants will be matched with a UW Integrated Care Training Program faculty member for monthly mentorship sessions.

Quality Improvement (QI)

Each participant will plan a quality improvement project to support the delivery of collaborative care principles in his or her current practice setting. Program support for this project will be provided in monthly QI calls as a cohort, QI core curriculum, monthly online assignments related to the QI project, and four in-person QI sessions.
